Dr. Jason Saylor spent 17 years in family medicine watching the same pattern repeat: a patient would see him for blood pressure, drive across town for a psychiatrist appointment, then wait weeks for a therapy referral somewhere else entirely. Three charts. Three offices. Zero coordination.
The result was predictable. Medications would conflict. Dosages would overlap. Patients would fall through the cracks between providers who never exchanged a single note. The system wasn't broken for lack of good doctors. It was broken because the doctors were in silos.
Ascend Mind and Body was Dr. Saylor's answer. He founded the practice on a straightforward principle: put primary care, psychiatry, and therapy under one roof, share one chart, and make it impossible for the left hand not to know what the right hand is doing.
What started as one office in Wesley Chapel now spans three locations - Wesley Chapel, Tampa, and Lakeland - with telehealth psychiatry and therapy available to patients anywhere in Florida. The team has grown to 9 providers, but the principle hasn't changed. Your provider team functions as a team, not as strangers who happen to share a patient.

Primary Care and Family Medicine
Annual physicals, chronic disease management, sick visits, labs - the foundation of your health. Led by Dr. Jason Saylor, DO, a board-certified family medicine physician with 17 years of experience. Available in Wesley Chapel and Tampa.
Psychiatry and Medication Management
Psychiatric evaluations, ongoing medication management, and ketamine-assisted therapy for treatment-resistant depression, anxiety, PTSD, and other conditions. Led by Anna Stouffer, PMHNP-BC, dual-board-certified with a decade of experience. In-person in Wesley Chapel or via telehealth across Florida.
Therapy and Counseling
Individual, couples, and family therapy. Trauma-informed and LGBTQIA+ affirming. Our Lakeland therapy team is led by Kaylee Mills Brenneman, LMHC, a licensed clinical supervisor overseeing a team of licensed and pre-licensed therapists. In-person in Lakeland and via telehealth statewide in Florida.
Coordination is the product
Any practice can offer primary care, psychiatry, and therapy. The thing that makes Ascend different is that our providers share records, share care plans, and talk to each other about your case. That's the actual product. The coordination.
Transparency about money
We publish our self-pay rates. We verify your insurance benefits within one business day. We tell you what you'll owe before you show up. We don't think surprise billing is acceptable and we don't do it.
Affirming care for everyone
Ascend is trauma-informed across every service line and every provider on the team. You don't have to screen us to find out if we're a safe place. We are.
Access over exclusivity
We accept Medicare, Medicaid, TRICARE, and ChampVA. We're actively credentialing with additional carriers. We offer telehealth statewide so geography isn't a barrier. And we provide superbills for out-of-network reimbursement when we're not yet in your plan.
Honest about scope
We don't do everything. We don't do surgery, emergency medicine, or inpatient care. When you need something outside our scope, we refer you to someone we trust and make sure the handoff is clean.

Anna Stouffer, PMHNP-BC - Lead Psychiatric Provider
Dual board-certified psychiatric and family nurse practitioner with a Master of Science in Nursing. Over a decade of psychiatric experience. Kaylee Mills Brenneman, LMHC - Clinical Supervisor, Therapy
Licensed Mental Health Counselor and Qualified Supervisor with an Ed.S and MEd. Kaylee leads the Lakeland therapy team, supervising both licensed and pre-licensed clinicians. Her team provides individual, family, and couples therapy with specializations in trauma, LGBTQIA+ issues, neurodiversity, and life transitions.
