At Ascend, Dr. Saylor evaluates each patient as part of your overall health - not as a standalone transaction. Schedule an Evaluation

Call or book online. If you’re an existing primary care patient, we can often add the evaluation to a scheduled visit.

Meet with Dr. Saylor

A thorough in-person evaluation to assess your qualifying condition, review your medical history, and discuss whether medical marijuana is appropriate for your care plan.

OMMU Registry

If you qualify, Dr. Saylor enters your certification into the Florida Office of Medical Marijuana Use registry. You’ll receive a registry ID card within about 5 business days.

Ongoing Care

Your certification is valid for 7 months. Dr. Saylor manages renewals and adjusts your treatment as needed - all coordinated with your other care at Ascend.

Florida law requires written informed consent before your first medical marijuana evaluation. You will complete the consent form at your appointment.

What conditions qualify for medical marijuana in Florida?

Florida recognizes cancer, epilepsy, glaucoma, HIV/AIDS, PTSD, ALS, Crohn’s disease, Parkinson’s disease, multiple sclerosis, chronic nonmalignant pain, terminal conditions, seizure disorders, and comparable conditions as determined by a qualified physician. Dr. Saylor will evaluate whether your condition qualifies during your appointment.

How much does a medical marijuana evaluation cost?

Contact our office at (813) 670-3331 for current evaluation and renewal pricing. Medical marijuana evaluations are not covered by insurance. We accept all major credit cards, HSA, and FSA.

Do I need to be a primary care patient to get evaluated?

No. You can schedule a medical marijuana evaluation as a standalone visit. However, patients who also receive primary care at Ascend benefit from coordinated treatment planning.

How long does the certification process take?

Your evaluation appointment typically takes 30–45 minutes. If you qualify, Dr. Saylor enters your certification into the OMMU registry the same day. You’ll receive your registry ID card within approximately 5 business days.

Is the consent form required before my visit?

Yes. Florida law (64B8-9.018, F.A.C.) requires written informed consent before a physician can certify a patient for medical marijuana. You will complete the consent form at your appointment or receive it from the office prior to your visit.
