Dr. Jason Saylor is a board-certified osteopathic family medicine physician with 17 years in practice. He trained at the Philadelphia College of Osteopathic Medicine and has been leading primary care at Ascend Mind & Body since the practice opened its doors. He also sees patients at our Tampa office on Moran Road, so scheduling flexibility is easy.

This is full-scope adult family medicine. Dr. Saylor treats you as a whole person, not a list of billing codes.
- Annual physicals and wellness visits - comprehensive exams with age-appropriate screenings, lab work, and a real conversation about how you are doing.
- Chronic disease management - hypertension, type 2 diabetes, high cholesterol, hypothyroidism. Regular labs, adjustments, and outcome tracking.
- Preventive care and screenings - USPSTF-aligned preventive screenings including cancer screenings, cardiovascular risk assessment, vaccinations.
- Acute sick visits - colds, flu, sinus infections, UTIs, minor injuries, skin concerns. When urgent care feels like overkill.
- Senior care - Medicare annual wellness visits, medication reconciliation, fall risk assessment, coordinated specialist referrals.
- Women’s primary care - well-woman exams, hormone screening, bone density, referrals for OB-GYN care.

What to Expect at Your First Wesley Chapel Visit
Before the visit. New-patient forms go out by email; fill them out at home. Bring your insurance card, photo ID, and a list of current medications. If you have records from a previous doctor, we request them in advance so Dr. Saylor is not starting blind.
During the visit. 45 minutes with Dr. Saylor. Medical history, physical exam, any labs that are due, and actual conversation about what you want out of a primary care relationship.
After the visit. Lab results post to our secure patient portal within a few business days. If follow-up is needed, the front desk schedules it before you leave.
The office is at 27724 Cashford Circle, Suite 102, Wesley Chapel, FL 33544. Free parking right out front, first-floor suite, no stairs or elevator required. Hours are Monday through Friday, 8:00 AM to 5:00 PM.
