If your relationship with alcohol or another substance has started costing you more than it gives back, and quitting has not been as simple as deciding to, that is not a moral failure. Substance use disorder is a recognized, treatable condition. Outpatient counseling can help, especially when there is anxiety, depression, or trauma underneath the use.

Short answer: Ascend provides outpatient counseling for substance use and treats the mental health conditions that so often come with it. We do not run a detox unit or a residential rehab, and we will say so plainly. When you need that level of care, we help you get to it.

Key Facts

  • It is a condition, not a character flaw: and it responds to treatment
  • Outpatient counseling: works well for many people, especially with co-occurring mental health conditions
  • Dual diagnosis: we treat substance use and mental health together, not in separate silos
  • Honest scope: detox, medication-assisted treatment, and residential rehab are coordinated by referral, not provided in-house

What Substance Use Disorder Is

Substance use disorder is a pattern of use that becomes compulsive and continues despite harm. It exists on a spectrum from mild to severe, and it changes the brain systems that govern reward, stress, and self-control. That is why "just stop" rarely works, and why effective treatment addresses the whole person, not only the substance.

The Co-Occurring Connection

For a large share of people, substance use travels with a mental health condition. Drinking to quiet anxiety. Using to numb depression or the symptoms of PTSD. When only one side is treated, the other tends to pull a person back. Co-occurring care, sometimes called dual diagnosis treatment, addresses both at once, which is exactly what an integrated mind and body practice is built to do.

What Outpatient Counseling Looks Like at Ascend

Outpatient counseling means regular therapy sessions while you continue living at home. The work often includes motivational interviewing to strengthen your own reasons for change, cognitive and behavioral skills for cravings and triggers, relapse-prevention planning, and treatment of the anxiety, depression, or trauma underneath the use. When medication for a co-occurring psychiatric condition is appropriate, our psychiatry team is part of the plan.

What We Coordinate Rather Than Provide

This is the honest boundary. Ascend does not operate a medical detox, a methadone or buprenorphine clinic, or a residential rehab. If you need medically supervised withdrawal, medication-assisted treatment for opioid or alcohol use, or 24-hour structure, those are the right tools, and we will connect you with trusted programs that provide them. Many people step down from those programs into outpatient counseling with us afterward. If you are in immediate danger or experiencing severe withdrawal, call 911.

Harm Reduction and Realistic Goals

Recovery is not one-size-fits-all. For some people the goal is abstinence. For others it starts with reducing harm and stabilizing. We meet you where you are, set goals you actually believe in, and adjust as you go. Judgment is not a treatment, and you will not get it here.

What to Expect

The first session is an assessment of your use, your history, your mental health, and your goals, plus a check on whether a higher level of care is needed first. From there you and your clinician build a plan, and we coordinate any medical or higher-level pieces. Progress is measured in stability, function, and the life you are trying to get back.

Frequently Asked Questions

Do you provide detox or medication-assisted treatment?

Not in-house. Ascend provides outpatient counseling and co-occurring psychiatric care. Medical detox, methadone or buprenorphine programs, and residential rehab are coordinated through trusted partners by referral. Many people use outpatient counseling with us as a step-down after those programs.

Do I have to be sober to start counseling?

No. You do not need to be abstinent to begin. We meet you where you are, and the first step is often figuring out the right level of care for your situation.

What is co-occurring or dual diagnosis treatment?

It means treating a substance use disorder and a mental health condition at the same time, rather than separately. Because the two so often drive each other, integrated care tends to work better than treating either alone.

Is online substance use counseling effective?

For many people, yes. Telehealth counseling removes travel and scheduling barriers and works well for outpatient care. If a higher level of care is needed, we help you find an in-person program.

Is what I share confidential?

Yes, your sessions are confidential and protected by law, with the standard limited exceptions such as an imminent risk of serious harm. Substance use treatment records carry additional federal protections. We will explain how your information is handled before you begin.

What if I relapse?

Relapse is a common part of recovery, not a reason for shame or for giving up. It is information that helps us adjust the plan. Relapse-prevention work is built into treatment from the start.
